Release of Liability and Transfer of Care

Authorization and Release of Liability, Participation and Safety

I hereby give permission to Ojassv Hatha Yoga to make available their services to my child. It is my understanding that the intention of Ojassv Hatha Yoga is to offer my child an opportunity to participate in a Yoga Camp Program. Programs are led by a responsible adult (minimum 18 years of age, however, where appropriate supervision takes place, the volunteer or staff member may be younger). I understand that all efforts will be made to select responsible facilitators/assistants/volunteers who will assist in the delivery of the group program.

In consideration of the privilege of my child's participation in the Camp Program by Ojassv Hatha Yoga, I hereby release, discharge, hold harmless and indemnify Ojassv Hatha Yoga, and the instructor Ms. Kritika Soni and volunteers of all responsibilities and liabilities in connection to their services provided in good faith, to myself or my child. I permit the agency to release any relevant information, including my personal information, to Ojassv Hatha Yoga insurers, as may be appropriate with any legal proceedings, inquiry or risk thereof.

I understand that the collection of personal information about myself or my child will be held in strict confidence and is to be used solely for the purposes of administering the program. I further agree that information about my child may be shared, at the discretion of Ojassv Hatha Yoga with the group facilitators so that my child's needs may be best met.

I understand that participation in the Camp Program may involve access to and the use of potentially harmful materials and supplies (e.g. scissors, glue, paint, etc.) which Ojassv Hatha Yoga may be providing to my child. I agree that my child is able to participate in these programs and activities. On behalf of my child, me, and my family, I assume these risks, dangers, and hazards with the above services and agree this is suitable for my child.

Consent to Medical Treatment and Release of Medical Information

I agree that if the facilitator(s) of Ojassv Hatha Yoga witness my child in distress, and they cannot reach the parent/guardian, emergency contact or designated caregiver, Ojassv Hatha Yoga will call 911. I further consent to the release of relevant medical information pertaining to my child collected by Ojassv Hatha Yoga to all medical personnel attending to my child. I understand that this is to ensure medical personnel are fully informed and possess all relevant information to provide the best medical attention and care possible for my child. I am responsible for payment of any medical charges or expenses not covered by my insurance.

All physical activities have risks. I agree to allow my child to participate in this program knowing there are physical risks and hazards inherent to any activity. I voluntarily agree to allow my child to participate in this program. By voluntarily allowing participation, I agree that I am exposing my child to inherent risks and hazards. I agree to accept all risks and hazards and be responsible for any injury or other loss which may occur during the participation of my child in this program.

I hereby agree to services being provided by Ojassv Hatha Yoga to my child. My signature below indicates that all information provided in this form is true and accurate, and that I fully agree to all statements made on this for, including but not limited to the Authorization and Release of Liability, Participation and Safety, Off-Site Permission, Consent to Medical Treatment, and Release of Medical Information. My signature also indicates that all legal guardians are aware and consensual with the participation of the above-named child.

Note: A printed copy of this waiver and agreement must be signed by parent/guardian at the time of camp check-in. By registering for camp, you acknowledge that you have read and agree to all terms outlined above.
